    Yes, this is a requirement for all applicants.

  2. Can I request the report prior to submitting my application for licensure?

    3. Yes, submit a request for a self-query report within 30 days of submitting the application for licensure.

  3. If I experience issues with completing the self-query or the self-query process, should I contact the Board?
